Preventive Services Reminder System (PSRS)

The Preventive Services Reminder System (PSRS) is a comprehensive clinical registry accessible through a secure, web-based practice portal. It utilizes a unique recommendation algorithm that considers patient demographics, past history of preventive services, evidence-based USPSTF, ACIP, ADA, AHA, and GOLD guidelines, personal allergies and risk factors, and service-specific modifiers (e.g., seasonal interventions) to calculate preventive services potentially indicated for individual patients. PSRS can also generate reminders to individuals eligible for particular services (e.g. varicella-zoster vaccine) or meeting certain age and gender criteria (e.g. older adults eligible for pneumococcal immunization).

My Wellness Portal & Health Risk Assessment (HRA) Tool

The My Wellness Portal HRA is an innovative and comprehensive personal wellness record that helps individuals improve their general well-being. It focuses on wellness, lifestyle, and preventive services and it facilitates patient-centered, continuous care through a "medical home". The Wellness Portal can help individuals maintain a record of their personal profile, history of preventive services, personal risk factors, care preferences, vital signs, symptoms, and medical encounters. It also provides personalized and prioritized care recommendations through an award-winning HRA tool, self-management support, health tracking, and secure messaging .

DartScreen Child&Teen Screener

The DartScreen Child & Teen Screener is a secure, mobile-device ready web application that includes three comprehensive health screening tools for children (4-10) and adolescents (11-14 and 15-19). Each screening tool includes 65-90 questions. Depending on which behavioral risk factors are reported, additional questions are also asked, including: demographics, family and medical history, personal health concerns, nutrition, physical activity and body image, school, home environment and safety issues, tobacco, alcohol and other drug use, sexuality and relationships, mental health and social issues. After the parent or the teen completes the screener on a touch-screen device, clinicians can assess responses through a personalized, risk-scored report for each area of pediatric health. A prioritized care approach can then be negotiated.
